MEDICAL VISA

1. Two (02) visa application forms duly completed, dated and signed by the applicant with two pictures attached to each form. Application forms can be downloaded from the consulate website.
2. A medical certificate covering medical expenses.
3. A letter of acceptance from the medical host institution.
4. A hotel reservation or an accommodation certificate duly certified by the local authorities in Algeria including the name, address, and, identification document of the inviting person.
5. An evidence of sufficient funds for treatment and accommodation expenses.
6. A passport valid for at least 6 months containing at least 2 blank pages for the issuance of the visa.
7. A copy of the first two pages of the passport.
8. A copy of a valid residence document in Turkey for non-Turkish applicants.
9. Visa fees.

Note: The Consulate reserves the right to request additional information/documentation if deemed necessary.
